Certified integration
Marketplace-listed, on the Partner API.
CCN Health is an official PointClickCare partner, listed in the PointClickCare Marketplace and built on the Partner API program. It's a fully sanctioned, two-way connection into the chart, not a screen-scrape or one-off export. Turning CCN on is a supported, low-lift configuration.

02Partner API
Built on PointClickCare's Partner API program: real-time and fully two-way.
03Low-lift config
A supported configuration turns CCN on. It's not a custom integration project.
- Listed in the PointClickCare Marketplace
- Built on the Partner API program
- Fully bidirectional, event-driven, real-time
- A supported configuration, not a screen-scrape or export
Two-way vitals synchronization
Vitals stay in lockstep between CCN and PointClickCare, both directions, no manual re-entry. Every reading CCN captures is written onto the resident's chart, tagged with the correct device type. And when a nurse charts a vital directly in PointClickCare, CCN ingests it within seconds.
- Eight vital types: BP, heart rate, SpO₂, temperature, weight, glucose, sleeping HR, respiratory rate
- Bidirectional, so device readings and nurse-charted vitals both sync
- Duplicate readings reconciled on both sides, never charted twice
- Struck-out observations in PointClickCare auto-remove from CCN
- A single source of truth: the same current, deduplicated vitals in either system
The benefit to your facility
One set of vitals, both directions, zero transcription. The nurse charts where she always charts, the devices chart themselves, and double-charting stops existing.
Three-way sync: facility EHR, practice EHR & CCN Health
In senior living, the facility runs on PointClickCare while a resident's medical care is often managed by an outside practice on its own EHR. CCN sits in the middle and keeps all three in lockstep: monitoring data lands in the PointClickCare chart AND the practice's EHR, and updates flow back, so nobody re-charts across systems.
- PointClickCare (facility) ⇄ CCN Health ⇄ the practice's EHR, all in lockstep
- A reading is captured once and written to both the facility and practice charts
- Connects the same way to any practice EHR: athenahealth, Epic, Charm Health, and Ethizo
- One source of truth across the facility, the practice, and CCN
The benefit to your facility
Nobody re-charts across systems, and nobody calls the facility to ask what the practice already knows. Every party charts where they already live, and all three systems stay honest.

Automatic resident census management
The roster manages itself off PointClickCare's admit, discharge, and transfer activity, the same events the facility already generates. No duplicate data entry, no roster drift.
- Admit / readmit / transfer → auto-create or reactivate the resident + pull the full profile
- Discharge handled per your workflow (flag as discharged, or move to a discharged roster)
- Cancelled admission → the record is removed cleanly, so there are never phantom patients
- One-click bulk import stands up a new site's entire active census in minutes
- Who's monitored in CCN always matches who's in the building
The benefit to your facility
No one maintains a second roster, ever. New residents are monitorable the day they arrive, departed residents stop billing the day they leave, and there are never phantom patients.
Active census · 48 residents
Matches who's in the building, no roster drift
Full clinical & insurance context, in-app
CCN pulls the surrounding clinical picture, not just vitals. A dedicated PointClickCare resident view lives inside CCN: demographics, room, primary payer, and conditions, with medications and ADT history, right where the monitoring work happens.
- Medications, conditions & diagnoses
- Insurance coverages
- Admit / discharge / transfer history
- A dedicated PointClickCare resident view, inside CCN
The benefit to your facility
Clinical context stops being a tab-switch away. Alert triage and care calls happen with the real medication and condition lists in view: faster reviews, safer decisions.

Chart facts become statuses & filters
CCN pulls information off the resident's chart, the primary payer with its effective date and the Medicare beneficiary identifier, and writes it onto the resident as CCN statuses: a chip named after the payer, and a Has beneficiary chip carrying the ID. Your team filters the whole building by those labels, on the patient list and every billing screen.
- Chart information pulled automatically and written as statuses under the resident's name
- The built-in pair: the primary-payer chip and the Has beneficiary chip with the Medicare ID
- Quick Filters, include or exclude, on the patient list and every billing screen
- Prior payer & effective dates kept; payer and beneficiary ID export in the billing CSV
- General by design: tell us which chart facts matter and we wire them into labels and filters
The benefit to your facility
Filtering a building by payer is how billing questions get worked: who is Medicare, who changed plans, who has an ID on file. Chart facts become labels your team acts on, with nothing re-keyed.

Contactless monitoring
For residents who can't or won't use cuffs and wearables, CCN adds a passive, contactless layer: radar and cloud sensors that read vitals from the room, with an early-warning decline score.
- Xandar Kardian contactless radar for passive HR & respiration, auto-activated the day after PointClickCare setup
- A nightly probability-of-clinical-decline score for early warning
- Neteera contactless heart rate: continuous HR, mapped to the resident
- No cuffs, no wearables. Vitals are captured from the room and tied to the chart
The benefit to your facility
The residents hardest to monitor are often the ones who need it most. Passive capture covers them with zero burden, and the decline score warns before anything is visible at the bedside.

Every connected device, wired to PointClickCare
CCN's full device catalog works in the PointClickCare context too. Every connected device is matched to the right resident automatically, provisioned as residents move in, and smart about not double-charting.
- Blood pressure, weight, glucose, CGM, pulse oximetry, temperature: the whole catalog
- DS Smart Bluetooth vitals, where CCN registers the device by serial and charts to PointClickCare
- Devices from Omron, Withings, Dexcom, Tenovi, BodyTrace, Smartmeter and more
- Auto-registered to the resident · no double-charting when PointClickCare is system of record
The benefit to your facility
One integration wires the entire device fleet to the building. Provisioning is a move-in step instead of a project, and the chart stays clean no matter how many vendors are in the mix.
