How it works
What happens
A reading gets taken in the room: a cellular device, a caregiver with the CCN app and a shared Bluetooth device, or a contactless sensor overnight. It travels to CCN Health on its own, gets checked against the resident's thresholds, and is written onto the PointClickCare chart tagged with the correct device type. And it works the other way too: when a nurse charts a vital directly in PointClickCare, CCN ingests it within seconds, so both systems always show the same current, deduplicated set.
How it works
CCN is built on PointClickCare's Partner API (connect2.pointclickcare.com). Device readings webhook into CCN, are matched to the resident, deduplicated, and posted as observations (POST /api/public/preview1/orgs/{org}/observations) with the device type on the record. In the other direction CCN reads the facility's charted observations (GET .../observations) and reconciles them: a same-minute duplicate is recognized and skipped rather than double-charted, and an observation struck out in PointClickCare is removed from CCN automatically.
The benefit to your practice
One set of vitals, both directions, no transcription in either system. The nurse charts where she always charts, the devices chart themselves, and the monitoring team, the facility, and the record all agree. Double-charting and copy-paste errors simply stop existing.
