How it works
What happens
In senior living, the building runs on PointClickCare while a resident's medical care is often managed by an outside practice on its own EHR. CCN sits in the middle: a reading captured once lands on the PointClickCare chart AND in the practice's EHR, and corrections flow back through, so the facility nurse, the practice provider, and the monitoring team are always looking at the same numbers.
How it works
CCN holds a live integration to each system: the PointClickCare Partner API on the facility side and the practice EHR's own integration (athenahealth, Epic, Charm Health, or Ethizo) on the practice side. The reading is stored once in CCN with the resident linked to both charts, then written to each through its own channel. Updates read back from PointClickCare (GET .../observations) reconcile against what CCN already holds before anything is re-written, so systems never ping-pong the same value.
The benefit to your practice
Nobody re-charts across systems, and nobody calls the facility to ask what the practice already knows. Every party charts in the system they already live in, and the integration keeps all three honest. This is the wiring that makes monitoring work when care is split between a building and a practice.
