Official integration
An official, OAuth-authorized connection.
CCN connects to Charm Health through Charm Health's official EHR API, an authorized, OAuth-based partner integration, not a screen-scrape or manual export. Data flows directly between CCN and the Charm Health chart with the practice's permission, so turning CCN on is a supported configuration, not a custom build.
01Official EHR API
CCN connects through Charm Health's official EHR API, not a screen-scrape or manual export.
02OAuth-authorized
An authorized, OAuth-based partner integration that the practice grants access to.
03Practice-permissioned
Data flows directly with the practice's permission. It's a supported configuration, not a custom build.
- Official Charm Health EHR API
- OAuth 2.0 authorized
- Two-way, with the practice's permission
- A supported configuration, not a screen-scrape
Vitals written into the Charm Health chart
Every reading CCN captures posts directly onto the patient's Charm Health vitals record in real time, with no manual transcription. Each value is recorded with its correct unit and the timestamp of the actual reading, in the practice's local time.
- Full remote-monitoring set: blood pressure (systolic & diastolic), pulse, SpO₂, temperature, weight, blood glucose
- Posted in real time, so the data lives in the clinician's primary chart, no double entry
- Each reading carries its correct unit and the reading's local-time timestamp
- Vitals-setup validation flags any type not yet configured, so a reading never silently fails to land
The benefit to your practice
Nobody transcribes a reading ever again. The clinician's primary record stays complete and current, and an out-of-range value becomes a nurse alert instead of a spreadsheet row.

Two-way secure patient messaging
Charm Health's standout capability. CCN is wired into Charm Health's bi-directional patient texting, so conversations between care staff and patients flow through and are captured in CCN, no longer stranded in a separate texting tool.
- Staff→patient and patient→team messages both captured, with the full content pulled from Charm Health
- Attributed to the individual staff member or the patient, with the original timestamp
- CCN can send outgoing messages through Charm Health too
- De-duplicated, so the whole exchange becomes part of the patient's history in CCN
The benefit to your practice
Two-way patient communication is the difference between monitoring and care management, and most integrations can't do it. Every conversation is on the record, attributable, and countable toward the interactive-communication requirements your billing depends on.
Automated monthly documentation, attributed to the right provider
CCN generates and files the monthly documentation a monitoring program produces, placing it directly into the Charm Health chart as patient documents, with no manual PDF export or upload.
- RPM reports: built and filed as an RPM document on the 1st of each month
- CCM care plans: filed as a CCM document on the 2nd, for chronic-care practices
- Each document matched from the patient's provider to their Charm Health team member
- Lands attributed to the right provider, not unassigned
The benefit to your practice
Month-end documentation drops from hours per patient to zero, filed on time and attributed to the right provider. Audit season becomes a non-event.

Streamlined patient onboarding & linking
Onboarding is fast and accurate, with demographics coming from the source of truth. Pick the facility, then bring in a patient one at a time, or import the whole facility's census at once.
- Facility selection: CCN pulls the practice's Charm Health facilities to scope the search
- Import individually by searching Charm Health by name within the facility, then link the chart
- Or import in bulk, pulling an entire facility's patients at once
- Already-imported detection means no duplicates, either way
- Direct import from the Charm Health record: create the patient and enroll into RPM or CCM in one step
The benefit to your practice
A panel stands up in minutes instead of weeks, with demographics that are right because they came from the record. Startup time collapses, and every later sync lands on the correct chart.

Clinical record access
Through the same connection, CCN retrieves the patient's Charm Health record to keep demographics current, and it's built to surface additional clinical context (medications and diagnoses) as that access is enabled.
- Demographics kept current from the Charm Health record
- Medications & diagnoses, available as access is enabled
- One authorized connection powers vitals, messaging, documents, and record access
The benefit to your practice
One authorized connection, granted once, powers everything: vitals in, messages both ways, documents filed, and a profile that never drifts from the chart. There is no second system to keep in sync by hand.
