How it works

What happens

All month, the monitoring work accrues in CCN: readings, calls, screenings, care plan updates. On the 1st, CCN builds the month's RPM report and files it into the patient's Charm Health chart as a patient document. On the 2nd, chronic-care practices get the CCM care plan filed the same way. Both land attributed to the right provider, with no PDF export and no upload.

How it works

CCN generates each document from the month's actual data, then files it through Charm Health's document endpoint. Before filing, it resolves the reviewer: the patient's CCN provider is matched to the corresponding Charm Health team member from the practice's member list, so the document arrives assigned rather than unattributed. Filing runs on schedule (RPM on the 1st, CCM on the 2nd) with no human in the loop.

The documentation a monitoring program owes the chart writes and files itself, on time, every month, attributed to the right provider. Audit season becomes a non-event, and nobody spends the first of the month exporting PDFs.
