HCPCS G2211: Office Visit Complexity Add-On, $16 Rate, Ongoing-Care Rules (2026).

HCPCS G2211 is the Medicare add-on code that recognizes the extra complexity of an evaluation and management visit when it is part of a continuing care relationship: the practitioner either serves as the ongoing focal point for all of the patient's care, or provides ongoing care for a single serious or complex condition. It attaches to office or outpatient E/M base codes 99202 through 99215 and, beginning in 2026, to home or residence E/M visits and qualifying telehealth visits, including audio-only. It is not a preventive code, so standard Part B cost-sharing applies.

2026 Reimbursement

$16.37

RVU: 0.49 × $33.4 CF

Time Requirement

No time threshold. G2211 pays for the nature of the care relationship, not additional minutes, so no extra time needs to be documented

Frequency

Once per qualifying E/M visit, with no annual cap. Every eligible visit in an ongoing relationship can carry it

Who Can Bill

The physician, NP, or PA who bills the base E/M visit and who is, or intends to be, the continuing focal point for the patient's care, or who manages a single serious or complex condition over time

Quick Answer

What is HCPCS G2211?

HCPCS G2211 is the Medicare visit complexity add-on, paying approximately $16.37 in 2026 alongside office or outpatient E/M visits (99202-99215) when the billing practitioner serves as the continuing focal point of the patient's care or manages a single serious or complex condition. It has no time threshold and no frequency cap. Since January 1, 2025 it is payable with a modifier-25 E/M on the same day as an Annual Wellness Visit, vaccine administration, or other Part B preventive service, and the 2026 fee schedule extends it to home, residence, and telehealth visits including audio-only.

Documentation

Documentation requirements.

01

A base office/outpatient E/M visit (99202-99215), or from 2026 a home/residence or qualifying telehealth E/M, documented and billable in its own right

02

The reason for the visit and its medical necessity, clearly stated in the note

03

Evidence of the longitudinal relationship in the record: prior visits, a care plan the practice owns, or documentation of intent to manage the patient's care going forward

04

For single-condition use: the serious or complex condition named, with the ongoing management the practice is providing

05

For same-day preventive stacking (2025 rule): the E/M with modifier 25 documented as distinct and non-duplicative of the AWV or preventive service

06

No separate time log and no special G2211 paragraph required: CMS sets no code-specific documentation elements beyond the base visit standards

07

Standard assessment and plan tying the visit to the continuing care relationship

Billing Workflow

How to bill G2211, step by step.

The calendar-month workflow a billing team actually runs — from qualifying the month to the increment math at month close.

01

Define who qualifies before the first claim

G2211 belongs on visits inside a continuing care relationship: the practice is the patient's medical home, or manages a serious or complex condition over time. Agree on that definition internally so front-line coders are not deciding it claim by claim.

02

Attach it at charge capture, not in retrospect

Add G2211 to the qualifying base E/M (99202-99215, plus home/residence and telehealth E/M from 2026) when the visit is coded. There is no separate documentation to assemble, so the decision point is charge entry.

03

Stack same-day preventive services correctly

When an AWV, vaccine administration, or another Part B preventive service shares the day with a distinct problem E/M, bill the E/M with modifier 25 and add G2211 to it. This combination has been payable since January 1, 2025.

04

Keep it off procedure-day claims

If the modifier-25 E/M exists because of a same-day minor procedure rather than a preventive service, leave G2211 off. This is the single most common G2211 denial pattern.

05

Tell patients about the cost-sharing

G2211 is not preventive, so the usual Part B deductible and coinsurance apply to it, roughly three dollars of coinsurance on a sixteen-dollar allowed amount. Practices that mention it avoid statement-driven complaints.

Scenarios

Common billing scenarios.

Established patient's hypertension follow-up with their continuing physician

Billable

A patient the practice manages long-term comes in for a routine hypertension and medication review with the physician who serves as the focal point of their care. The E/M (for example 99214) qualifies for G2211.

One-off urgent visit with no ongoing relationship

Not Billable

A patient is seen once for an acute problem at a walk-in slot with no intent to assume ongoing care. The relationship requirement is not met, so G2211 does not apply even though the base E/M is billable.

Problem E/M with modifier 25 on the same day as a subsequent AWV (G0439)

Billable

Since January 1, 2025, when the same practitioner furnishes the AWV and a distinct, separately identifiable E/M with modifier 25 on the same day, the E/M can also carry G2211. Before 2025 the modifier-25 restriction blocked this.

E/M with modifier 25 on the same day as a minor procedure

Not Billable

The 2025 allowance covers only same-day AWV, vaccine administration, and other Part B preventive services. An E/M reported with modifier 25 because of a same-day minor procedure, such as a joint injection or skin biopsy, still cannot carry G2211.

Audit & Compliance

Audit rules & denial defense for G2211.

What a payer reviewer checks on a records request, the failure that triggers each denial, and the documentation that survives it.

The continuing care relationship is visible in the record

Denial risk

Claims reviews look for evidence the practitioner is the ongoing focal point or manages the named condition over time. A chart showing a single isolated encounter invites recoupment.

What survives review

Prior-visit history, an active care plan, or an explicit note of intent to assume ongoing care, plus the condition being managed where the single-condition pathway is used.

The base code is eligible

Denial risk

G2211 attached to anything other than 99202-99215, or from 2026 the home/residence E/M and qualifying telehealth codes, denies automatically.

What survives review

Charge-capture rules that only offer G2211 on eligible base codes, so the pairing can never be keyed on an ineligible claim.

Modifier-25 context matches the 2025 policy

Denial risk

G2211 with a modifier-25 E/M is payable only when the same practitioner furnished an AWV, vaccine administration, or another Part B preventive service that day. Procedure-day modifier-25 claims still deny.

What survives review

The same-day preventive service on the claim, and an E/M note that is distinct and non-duplicative of it, per MLN Matters MM13473.

No duplication with the visit itself

Denial risk

Reviewers check that the add-on reflects relationship complexity rather than re-billing work already valued in the base E/M.

What survives review

A note that stands on its own for the base code, with the longitudinal context (medication titration over visits, coordination the practice owns) evident rather than restated boilerplate.

FAQ

Common questions about HCPCS G2211.

01

What is HCPCS G2211?

G2211 is the Medicare add-on code for visit complexity. It pays approximately $16.37 in 2026 on top of an office or outpatient E/M visit when the billing practitioner is, or intends to be, the continuing focal point for the patient's care, or is managing a single serious or complex condition over time.

02

How much does G2211 reimburse in 2026?

Approximately $16.37 nationally under the 2026 Medicare Physician Fee Schedule, from a total RVU of 0.49 and the $33.40 conversion factor, before locality adjustment. The work RVU, 0.33, has been unchanged since the code became payable in 2024. Standard Part B deductible and coinsurance apply.

03

Can G2211 be billed with G0439?

Yes, since January 1, 2025. When the same practitioner furnishes an Annual Wellness Visit (G0438 or G0439) and a distinct, separately identifiable problem E/M with modifier 25 on the same day, the E/M can also carry G2211. CMS made the change in the CY2025 Physician Fee Schedule final rule (MLN Matters MM13473); before 2025, any modifier-25 E/M was excluded. The AWV itself never carries G2211.

04

When can G2211 not be billed?

On one-off visits with no ongoing care relationship, on E/M visits reported with modifier 25 because of a same-day minor procedure, by practitioners billing within a surgical global period, and on any base code outside the eligible E/M set. The 2025 modifier-25 allowance covers only same-day AWVs, vaccine administration, and other Part B preventive services.

05

What changed for G2211 in 2026?

The CY2026 Physician Fee Schedule extends G2211 beyond the office: it can now accompany home or residence E/M visits (99341-99345 range) and qualifying telehealth visits, including audio-only. The rate stays approximately $16.37 and the relationship requirement is unchanged.

06

Does G2211 require extra documentation?

No code-specific documentation elements exist. CMS expects the base visit note to state the reason for the visit and its medical necessity, with the continuing care relationship evident from the record. There is no time threshold and no separate attestation, which is why consistent charge capture, not documentation burden, decides how much G2211 revenue a practice actually collects.
