Adding a second device to an existing RPM patient

Not Billable

A patient already enrolled in RPM with a blood pressure cuff now also needs weight monitoring. A scale is provided and set up. Because 99453 is billed per episode of care (not per device), it cannot be billed again for adding a device to an active RPM patient.

Re-enrollment after a gap in monitoring

Billable

A patient was discharged from RPM six months ago and is now re-enrolling with new devices for a different condition. A new episode of care has been established with a new treatment plan. CPT 99453 can be billed for this new episode.

Setup using a consumer fitness tracker

Not Billable

A practice sets up a patient with a consumer-grade smartwatch for heart rate tracking. Because the device is not FDA-cleared as a medical device, 99453 cannot be billed regardless of the education provided.
