CCN Health Releases Mobile App for Bedside Vital Sign Monitoring
LOS ANGELES, CA — August 13, 2025 — CCN Health today released its mobile application for iOS and Android, giving nurses and clinical staff a dedicated tool for taking Bluetooth vital sign readings at the bedside. The app supports six device types, organizes patients by room and facility, and syncs readings directly to the CCN Health platform and connected EHRs in real time.
Bluetooth Device Pairing at the Point of Care
The app connects to FDA-cleared Bluetooth monitoring devices including blood pressure cuffs, pulse oximeters, weight scales, thermometers, blood glucose meters, and continuous glucose monitors (CGM). Nurses select a patient, choose a device type, and the app initiates a Bluetooth scan. Once paired, the reading captures automatically — no manual data entry required.
This replaces the workflow where staff would take a reading on a standalone device, write the value on paper or a whiteboard, and later enter it into the EHR. With the mobile app, the reading flows from the device to the patient's clinical record in a single step.
Patient Management by Room and Facility
The app displays patient lists organized by name, room number, and monitoring status. Staff can see at a glance which residents are due for readings (Pending), currently being monitored (In Check), or up to date. A search function and sort options (by name, room, or last reading date) help nurses navigate large censuses efficiently.
For organizations operating across multiple locations, the app supports facility switching. A nurse covering shifts at two assisted living communities can toggle between them without logging out or re-authenticating — each facility loads its own patient roster, room assignments, and monitoring protocols.
Clinical Notes and Documentation
Beyond device readings, the app includes a clinical notes feature. Nurses can add timestamped notes to any patient record — documenting observations, care plan changes, or follow-up actions. Notes can be added standalone or attached to a specific reading for context. A history view shows all notes across configurable time ranges (1 week, 1 month, 3 months, 6 months).

Real-Time Platform Sync
Every reading taken through the app syncs immediately to the CCN Health cloud platform. From there, automated workflows handle the rest: alert threshold evaluation, EHR posting via existing integrations (PointClickCare, Epic, athenahealth, ALIS, and others), billing documentation generation, and compliance tracking across RPM, CCM, PCM, BHI, and RTM programs.
