LOS ANGELES, CA — November 8, 2023 — CCN Health today announced that its Remote Patient Monitoring, Chronic Care Management, and related Medicare programs now operate in 25 states. The geographic expansion reflects growing demand from both senior living operators managing multi-state facility portfolios and physician practices seeking to add remote monitoring revenue.
Geographic Concentration
The highest concentration of CCN Health partners is in California, Florida, and Texas — the three states with the largest Medicare beneficiary populations. Pennsylvania and New York follow, reflecting strong adoption among senior living communities and internal medicine practices in the Northeast.
Additional states in the Southeast, Midwest, and Mountain West regions round out the 25-state footprint. The expansion pattern largely follows Medicare beneficiary density and the presence of multi-site senior living operators whose portfolios span multiple states.
