CCN Health Surpasses 10,000 Active Monitored Patients
LOS ANGELES, CA — July 16, 2025 — CCN Health today announced that its platform now monitors more than 10,000 active patients across its RPM, CCM, PCM, BHI, and RTM programs. The milestone represents a doubling of the company's monitored patient census in approximately seven months, up from 5,000 patients at the end of 2024.
Growth Drivers Behind the Milestone
Three factors drove the acceleration in patient enrollment. First, achieving 50-state RPM coverage in February 2025 removed geographic barriers that had previously limited the company's addressable market. Multi-site operators that had been unable to standardize on CCN Health across all their locations began consolidating onto the platform.
Second, the company's expanding portfolio of EHR integrations — now including PointClickCare, ALIS, athenahealth, Epic, Charm Health, August Health, Ethizo, and MatrixCare — reduced the technical friction of onboarding new facilities. Automated data exchange with these systems means that new patient enrollments flow through existing clinical workflows rather than requiring parallel documentation.
Third, multi-program enrollment allowed existing patients to be enrolled in additional Medicare programs without duplicating the onboarding process. A patient already on RPM for blood pressure monitoring could be simultaneously enrolled in CCM for care coordination, increasing per-patient program participation and improving clinical outcomes.
The Scale of Operations
At 10,000 active patients, the platform processes tens of thousands of device readings per day across blood pressure monitors, weight scales, glucose meters, pulse oximeters, continuous glucose monitors, and contactless radar sensors. The clinical operations team manages hundreds of alerts daily, triaging readings that fall outside patient-specific thresholds and coordinating interventions with on-site care teams.
The growth has been supported by investments in clinical staffing, platform infrastructure, and automated workflows that handle routine monitoring tasks — allowing clinical staff to focus their time on patients who need direct attention rather than manual data review.
