Why we built this
Most monitoring time goes not to care itself, but to deciding what each patient needs and hunting for the next one.
We did a deep analysis of how monitoring teams actually spend their day, and that decide-and-search overhead (not the care itself) was the bottleneck. So we hyper-optimized it away: CCN spots which patients need what, groups them by need, and hands your team an ordered queue. Triage becomes throughput, saving real time and money.
Every night, we do the triage for you.
While your team is offline, CCN analyzes every patient, grades them against every workflow, and buckets them by unmet need. By the time you log in, the work is already sorted and queued, with no morning scramble to figure out who needs what.
- Runs automatically every night, across your whole panel
- Every patient graded against every workflow, so nothing gets missed
- Live per-workflow counts, ready the moment you log in
- You start the day with full, prioritized queues

The Tasks bar
Your whole day, already bucketed. A strip of Task cards sits atop the patient panel (one per workflow), each with a live count and a status, so the work comes to the monitor instead of the monitor hunting for it.
- One card per workflow, with an icon, title, a big live count, status & progress bar
- Cards change state so your eye goes to the work: idle · active (blue glow) · completed (check)
- The counts read “Unseen Readings 34 · Out of Range 8 · Monitoring Time 51 · Communication 22”
- Click a pile → the panel filters to it, a blue banner confirms the scope, and Begin Workflow launches the lane

Every workflow, pre-computed
The platform has already enumerated the kinds of work that exist and graded every patient against them, so “open 2,480 charts to find the 96 that are out of range” becomes a card that says “Out of Range · 96,” a ready-made queue of exactly those patients.
- RPM: Unseen Readings, Reading Reminders, Monitoring Time, Out of Range, Communication, Out of Range POBC, Needs Consent + CPT gaps (99453 / 99454 / 99457)
- CCM / PCM: Care Plan, Needs Time, Communication, Ready to Finalize, Screenings
- Each name is literally a thing a monitor is trying to do, pre-computed across the whole panel
- No spreadsheets, no deciding who's next: the buckets are already counted

The guided lane
One patient at a time, everything teed up. Begin Workflow builds a queue of exactly the patients who share that need (the 96 who are out of range) and walks you through them one after another, doing the same thing to each.
- A queue of exactly the patients who share the need: you're on Patient 35 of 96
- One clear action per patient. Resolve, and CCN loads the next chart automatically
- The same task for every patient in the queue, with no deciding and no searching
- Your place, the running timer, and the sort order all follow you from patient to patient

The chart anticipates the task
See why, fix it, move on. The chart reshapes around the workflow: in the Out-of-Range lane the header expands to the full out-of-range table with one-tap Resolve / Resolve With Note.
- The timer auto-starts on load and auto-submits when you advance, so capture is continuous with zero manual entry
- One-tap resolution clears alerts; Resolve With Note auto-writes the documentation
- Quick-Note chips one-tap canned notes; a “two-way communication” toggle flags it as a compliance call
- The amber Sticky Note refreshes per patient, so hand-off context never gets lost

A clear finish, then the next pile
On the last patient, “next” auto-routes to an “All Done!” screen (100% bar, “All 8 patients reviewed,” Back to Patients), where that task card now reads “all done.” Loop closed, next pile visible.
- A completion screen confirms the bucket is empty, with the time and evidence already captured
- Back to Patients → the Tasks bar shows the cleared pile and the next one to attack
- The monitor works by category of need instead of patient-by-patient, and it's documented automatically

Work with our team to customize your workflows.
Every practice monitors a little differently. Work with our team to tailor the buckets, thresholds, sort order, and lanes to how you actually operate, or design entirely new workflows around a need we haven't bucketed yet. Your workflows, shaped around your operation.
What we tailor with you
- Custom buckets & triggers for the needs you track
- Thresholds & priorities tuned to your protocols
- Sort & queue order that matches how your team works
- Brand-new lanes for workflows we haven't built yet
