How it works
What happens
A patient takes a reading at home on a cellular device: no wifi, no phone, no Bluetooth pairing. The reading travels to CCN Health on its own over the cellular network, gets checked against that patient's alert thresholds the moment it arrives, and is then posted into their Epic chart, where the care team sees it alongside everything else in the record. The same numbers appear in both systems, so the monitoring team and the care team are always reading from the same page.
How it works
In FHIR R4, a single measurement is an Observation resource: one structured record holding the value, its units, and the exact time it was taken. When a reading arrives, CCN Health matches it to the patient, screens it against thresholds (a breach triggers a clinical alert for nurse review), then posts it to the Epic chart as an Observation tied to that patient's Patient resource, which is how it lands on the correct chart every time. When the clinical work around the readings is documented, the progress note is written back to the record as a DocumentReference resource, the FHIR way of filing a clinical note against the chart, and the underlying time logs and transmission records compile into monthly billing reports with CPT codes 99453, 99454, 99457, and 99458.
