The patient experience

Freedom from fingersticks.

The sensor is applied to the back of the upper arm with a one-press applicator and worn for 10 to 15 days. Factory calibrated: no fingersticks, no coding, and nothing to sync by hand.

In real life

The complete glucose picture.

Where traditional meters provide 2 to 4 data points per day, a CGM generates up to 1,440: the patterns that spot-checks miss, streamed to your dashboard.

How readings flow

From the sensor to your dashboard.

CGM data streams to the CCN Health platform in real time. Configurable thresholds trigger alerts for hypo- and hyperglycemia so your clinical team can act between visits.

01

Apply the Sensor

A small, water-resistant sensor is applied to the back of the upper arm. The auto-applicator makes insertion quick and virtually painless.

02

Continuous Tracking

The sensor measures glucose every 1 to 5 minutes, streaming real-time data to the patient's device and the CCN Health platform.

03

See and Act on Data

Patients and clinicians view trends, set customizable high/low alerts, and make informed treatment decisions based on continuous data.

Questions practices ask

The answers, straight.

Yes. Dexcom continuous glucose monitors are FDA-cleared devices that qualify for RPM billing under CPT 99454. Glucose readings are captured every 5 minutes and transmitted to the CCN Health platform for clinical review.

Dexcom sensors transmit glucose readings to the patient's smartphone app, which syncs with the Dexcom cloud. CCN Health pulls data from the Dexcom API, providing clinical staff with real-time glucose trends, time-in-range metrics, and alert notifications.

Dexcom provides 288 readings per day (every 5 minutes) from a sensor worn for 10-15 days, versus 1-4 daily finger-stick readings with a traditional meter. CGM reveals glucose patterns, overnight trends, and postprandial spikes that finger sticks miss.

Type 1 diabetes, Type 2 diabetes on insulin therapy, gestational diabetes, and patients with hypoglycemic unawareness benefit most. The continuous data stream enables precise medication titration and early detection of dangerous glucose excursions.

Yes. Dexcom CGMs require a compatible smartphone for data relay. This differs from cellular RPM devices that transmit independently. For patients without smartphones, CCN Health offers standalone cellular glucose meters as an alternative.

The rest of the fleet

One platform, eleven device categories.

Continuous Glucose is one piece. Cellular scales, glucose meters, pulse oximeters, sleep sensors, and more all feed the same dashboard, the same alerts, and the same billing engine.
