Yes. Dexcom continuous glucose monitors are FDA-cleared devices that qualify for RPM billing under CPT 99454. Glucose readings are captured every 5 minutes and transmitted to the CCN Health platform for clinical review.
Dexcom sensors transmit glucose readings to the patient's smartphone app, which syncs with the Dexcom cloud. CCN Health pulls data from the Dexcom API, providing clinical staff with real-time glucose trends, time-in-range metrics, and alert notifications.
