CCN Health Introduces Remote Therapeutic Monitoring Program
LOS ANGELES, CA — September 13, 2023 — CCN Health today announced the launch of its Remote Therapeutic Monitoring program, extending the company's Medicare program portfolio to include therapy outcome tracking for musculoskeletal and rehabilitation patients. The RTM program enables orthopedic practices, physical medicine providers, and rehabilitation specialists to monitor patient progress remotely and bill Medicare under CPT codes 98975 through 98981.
How RTM Differs from RPM
While RPM captures physiological data — blood pressure, weight, glucose, and oxygen saturation — RTM focuses on therapy and treatment outcomes. RTM tracks metrics such as pain levels, medication adherence, functional mobility, and rehabilitation progress. This distinction is important because it opens remote monitoring to a category of patients and conditions that do not generate the physiological device data required for RPM.
Patients recovering from joint replacement surgery, managing chronic lower back pain, or progressing through a physical therapy protocol can now be monitored remotely through structured outcome assessments rather than connected medical devices.
Conditions Served
The RTM program supports musculoskeletal conditions including post-surgical rehabilitation following knee, hip, and shoulder procedures. Chronic pain management patients benefit from systematic tracking of pain scores, functional assessments, and medication response over time. Physical therapy patients can report range of motion progress, exercise compliance, and symptom changes between office visits.
The program also serves patients with chronic musculoskeletal conditions such as osteoarthritis, degenerative disc disease, and fibromyalgia, where ongoing outcome monitoring helps clinicians adjust treatment plans based on longitudinal trend data.
Expanding to New Specialties
The addition of RTM extends CCN Health's reach into orthopedic surgery, physical medicine and rehabilitation, and sports medicine practices that previously had no Medicare remote monitoring option.
