CCN Health Launches Real-Time Provider Dashboard with Population Health Views

LOS ANGELES, CA — June 4, 2025 — CCN Health today released a real-time provider dashboard that gives clinical teams and administrators a consolidated view of their entire monitored patient population. The dashboard combines real-time census data, alert summaries, compliance tracking, and program enrollment metrics into a single interface designed for both individual patient management and population-level oversight.

Population Health Meets Individual Patient Detail

The dashboard operates at two levels. Population health views show aggregate metrics across all monitored patients — total active enrollments by program, alert volumes by severity, compliance rates for device usage and clinical time thresholds, and trending vital sign patterns across the census. Administrators can filter these views by facility, care setting, program type, or clinical team to isolate the metrics that matter most to their operations.

Individual patient views are accessible from any population-level metric. Clicking on an alert count drills down to the specific patients generating those alerts. Selecting a compliance metric reveals which patients are approaching or missing their monthly device transmission requirements. This two-level design lets clinical staff move fluidly between monitoring their overall program health and addressing individual patient needs.

Customizable Widgets for Multi-Facility Operators

For operators managing multiple facilities, the dashboard supports customizable widget layouts that can be configured per user role. A corporate administrator might prioritize cross-facility enrollment trends and revenue metrics, while a facility-level nurse manager focuses on today's alerts and pending clinical reviews. Widget configurations are saved per user and persist across sessions.

The dashboard also includes program enrollment funnel metrics that track patients from initial eligibility identification through device setup, first reading, and ongoing compliance. These metrics help operators identify where patients are dropping out of the enrollment process and where operational bottlenecks are slowing program growth.
