The integration connects CCN Health's remote patient monitoring and chronic care management platform directly to MatrixCare via secure API, enabling automated bidirectional data exchange between clinical workflows.
Integration Capabilities
The MatrixCare integration delivers several operational efficiencies for post-acute care operators:
- Resident Demographic Sync — Patient demographic data flows automatically from MatrixCare into CCN Health, eliminating manual data entry and reducing transcription errors during enrollment.
- Automated Vital Sign Posting — Device readings collected through CCN Health's monitoring programs are posted directly into the MatrixCare clinical record, giving nursing staff real-time visibility without switching platforms.
- ADT Event Monitoring — Admission, discharge, and transfer events in MatrixCare trigger automatic updates within CCN Health, keeping enrollment rosters current and ensuring continuity of monitoring across care transitions.
- Billing Documentation Generation — Time-based billing records for RPM, CCM, PCM, BHI, and RTM programs are generated automatically, with documentation mapped to MatrixCare's billing workflows.
