The REST API integration connects CCN Health's remote monitoring and chronic care management programs directly to the Charm Health clinical record.

The Charm Health integration delivers automated data exchange across several key workflows:
- Patient Chart Data Sync — Patient demographics and clinical data flow from Charm Health into CCN Health, enabling rapid enrollment without manual chart abstraction.
- Device Reading Documentation — Vital signs and biometric data captured through CCN Health's monitoring devices are posted directly into Charm Health patient charts, providing clinicians with trending data inside their existing EHR workflow.
- Care Coordination Notes — Clinical notes from CCM, PCM, and BHI program activities are synced to Charm Health, ensuring the patient record reflects all care management interactions without requiring duplicate documentation.
- Claims-Ready Billing Records — Time-tracked billing documentation for RPM, CCM, PCM, BHI, and RTM programs is generated automatically and formatted to support Charm Health's billing and claims submission processes.
