CCN Health Adds Continuous Glucose Monitoring to Its Platform
LOS ANGELES, CA — October 4, 2023 — CCN Health today announced support for continuous glucose monitoring devices on its remote monitoring platform. CGM integration enables endocrinology practices, internal medicine groups, and senior living operators to receive real-time glucose trend data for patients with diabetes, providing a significant clinical advantage over traditional fingerstick glucose monitoring.
CGM Advantages Over Fingerstick Monitoring
Traditional glucose meters capture a single point-in-time reading when the patient performs a manual fingerstick test. CGMs, by contrast, measure interstitial glucose levels continuously, generating readings every few minutes throughout the day and night. This continuous data stream reveals glucose patterns that isolated fingerstick readings cannot capture.
With CGM data flowing into the CCN Health platform, clinicians can identify nocturnal hypoglycemia episodes that patients may not be aware of, detect post-meal glucose spikes that indicate dietary issues, and track overall time-in-range metrics that reflect glycemic control more accurately than periodic A1C lab tests.
How CGM Data Flows into Monitoring Programs
CGM sensor data transmits to the CCN Health platform where it integrates with the patient's RPM or CCM record. The platform applies clinical thresholds to the continuous data stream, generating alerts when glucose levels fall below hypoglycemia thresholds or exceed hyperglycemia limits.
Daily glucose trend summaries are generated automatically, giving clinicians a consolidated view of each patient's glycemic patterns without requiring them to review thousands of individual data points. These summaries support the clinical review documentation required for RPM billing and CCM care plan updates.
Endocrinology and Senior Living Use Cases
For endocrinology practices, CGM support enables remote management of insulin-dependent Type 1 and Type 2 diabetes patients between office visits. Clinicians can adjust insulin dosing and medication regimens based on actual glucose trends rather than waiting for quarterly lab results.
