CCN Health Launches Behavioral Health Integration Program

LOS ANGELES, CA — June 7, 2023 — CCN Health today announced the launch of Behavioral Health Integration as its third Medicare program, expanding beyond RPM and CCM into psychiatric collaborative care. The BHI program enables physician practices and senior living operators to deliver structured behavioral health monitoring and bill Medicare under CPT 99484.

Addressing the Behavioral Health Gap

Medicare beneficiaries experience depression, anxiety, and substance use disorders at rates that often go unaddressed in primary care and senior living settings. While RPM captures physiological data and CCM coordinates chronic disease management, neither program was designed to address the behavioral health needs that frequently accompany chronic medical conditions.

BHI fills this gap by creating a structured framework for behavioral health screening, care plan development, and ongoing psychiatric monitoring. Qualifying conditions include major depressive disorder, generalized anxiety disorder, substance use disorders, PTSD, and other behavioral health diagnoses documented in the patient's medical record.

How BHI Complements RPM and CCM

For patients already enrolled in RPM or CCM, adding BHI creates a more complete picture of their health status. A patient being monitored for hypertension through RPM may also carry a depression diagnosis that affects medication adherence, activity levels, and clinical outcomes. BHI allows the care team to address both dimensions within a single coordinated program.

The CCN Health platform tracks BHI encounters separately from RPM and CCM, maintaining distinct time logs and clinical documentation for each program. This separation ensures clean billing compliance while allowing clinicians to view the patient holistically across all active programs.

CPT 99484 Billing Structure

CPT 99484 reimburses for 20 or more minutes of behavioral health care manager time per calendar month, delivered under a psychiatric collaborative care model. The code requires an initial psychiatric assessment, a measurable treatment plan, and systematic tracking of patient outcomes using validated rating scales.

Availability

The BHI program is available now to all CCN Health customers. Practices serving Medicare patients with documented behavioral health conditions can enroll eligible patients immediately.
