CCN Health Surpasses 5,000 Patients Under Monitoring
LOS ANGELES, CA — December 11, 2024 — CCN Health today announced that its platform has surpassed 5,000 patients under active monitoring across all five Medicare care management programs. The milestone, reached eight months after crossing the 3,000-patient mark, reflects sustained growth in partner facility adoption and program enrollment depth.
Growth Across All Five Programs
The 5,000-patient milestone spans the full breadth of CCN Health's program portfolio. Remote Patient Monitoring and Chronic Care Management continue to represent the largest enrollment categories, while Principal Care Management, Behavioral Health Integration, and Remote Therapeutic Monitoring have grown as providers become more sophisticated in matching patients to the program that best fits their clinical profile.
The growth has been distributed across care settings, with senior living communities, skilled nursing facilities, and physician practices all contributing to enrollment increases. Multi-site operators that launched programs at individual facilities in 2023 have expanded across their portfolios, adding locations in batches as operational teams develop proficiency with the platform.
Clinical Outcomes at Scale
With 5,000 patients generating continuous monitoring data, CCN Health's clinical outcomes data has reached a scale that supports meaningful analysis. Across the monitored patient population, the platform has observed a measurable reduction in emergency room visits among enrolled patients compared to pre-enrollment baselines. Medication adherence rates have improved as well, driven by the regular clinical touchpoints that care management programs require.
Blood pressure control rates among hypertensive patients enrolled in RPM have shown sustained improvement over the first six months of monitoring. Weight monitoring programs for heart failure patients have enabled earlier detection of fluid retention, allowing clinical teams to intervene with medication adjustments before acute decompensation events occur.
Partner Facility Growth
The path from 3,000 to 5,000 patients involved both new partner acquisitions and deeper penetration within existing partner organizations. Several senior living operators have moved from monitoring a subset of eligible residents to offering enrollment to their full qualifying population. This shift reflects growing confidence in program value as operators observe clinical outcomes and financial performance data from their initial cohorts.
"Five thousand patients is a meaningful proof point, but what matters more is the clinical data behind the number," said Cosmo Cochrane, Co-Founder of CCN Health. "We are seeing reduced ER visits, better medication adherence, and earlier clinical interventions across every program. That is the metric we are building toward."
Looking Ahead to 2025
CCN Health enters 2025 with operational infrastructure scaled for continued growth, a device ecosystem spanning seven monitoring categories plus contactless and fall detection, and coverage in 40 states. The company plans to deepen EHR integrations, expand its clinical team, and extend its reach to new care settings and geographic markets throughout the year.
