CCN Health Achieves Nationwide RPM Coverage Across All 50 States
LOS ANGELES, CA — February 19, 2025 — CCN Health today announced that its remote patient monitoring platform now operates across all 50 states, completing a multi-year expansion from its initial markets in California, Florida, and Texas. The milestone establishes CCN Health as a nationwide RPM and chronic care management provider capable of serving multi-site operators with facilities spread across multiple state jurisdictions.
From Regional Roots to Nationwide Operations
CCN Health launched its RPM platform in three states and expanded methodically, adding state-level regulatory compliance for telehealth consent, clinical licensure, and Medicaid program requirements in each new market. The expansion required building a compliance framework that accounts for the variation in telehealth regulations across state lines — from consent-to-treat requirements to scope-of-practice rules for remote clinical staff.
The operational infrastructure behind nationwide coverage includes distributed clinical teams, state-specific documentation workflows, and automated compliance checks that verify program eligibility based on each patient's state of residence. These systems run in the background so that facility operators and physician practices experience a single, consistent platform regardless of where their patients are located.
What Nationwide Coverage Means for Multi-Site Operators
For senior living chains and physician practice groups operating across multiple states, 50-state coverage eliminates the need to contract with different RPM vendors in different regions. A skilled nursing operator with facilities in Ohio, Pennsylvania, and North Carolina can run all three locations through a single CCN Health implementation, with unified reporting, shared clinical protocols, and consolidated billing.
"Reaching 50-state coverage is not just a geographic milestone — it is an operational one," said Cosmo Cochrane, Co-Founder of CCN Health. "Multi-site operators told us that managing different RPM vendors in different states created fragmentation that undermined the whole point of centralized monitoring. Now they can standardize on one platform everywhere."
The expansion also positions CCN Health to support state Medicaid RPM programs, which vary significantly in covered services, eligible populations, and reimbursement rates. The platform's state-specific configuration layer adapts billing and documentation workflows to match each state's Medicaid requirements where applicable.
Availability
Fifty-state RPM and CCM coverage is available now to all CCN Health customers. Multi-site operators can onboard new state locations through their existing account infrastructure.
