LOS ANGELES, CA — August 21, 2024 — CCN Health today announced that its remote patient monitoring and care management platform now serves patients in 40 states, up from 25 states at the beginning of the year. The expansion has been driven primarily by multi-site senior living operators extending successful programs across their facility portfolios and by new physician practice partnerships in previously unserved regions.
Geographic Growth
The 15 new states added in 2024 span the Midwest, Mountain West, and Southeast regions, reflecting the geographic distribution of CCN Health's growing partner base. Several multi-site senior living operators with facilities in 10 or more states have been a primary driver of this expansion, as organizations that launch RPM programs at a handful of locations typically move to enterprise-wide rollouts within six to twelve months of initial results.
The expansion also includes new physician practice partnerships in states where CCN Health had not previously operated. Internal medicine groups, cardiology practices, and endocrinology practices in these regions are adopting RPM and CCM programs as the clinical and financial evidence base for remote monitoring continues to strengthen.
Navigating State Regulations
Operating across 40 states requires navigating a patchwork of state-level regulations governing telehealth, remote monitoring, and care management billing. While RPM and CCM are Medicare programs with federal reimbursement structures, state-level requirements around provider licensing, patient consent, and data privacy vary meaningfully.
