LOS ANGELES, CA — April 24, 2024 — CCN Health today announced that its platform has reached 3,000 active patients under monitoring across all five Medicare care management programs. The milestone represents a threefold increase in enrollment in under a year, driven by expansion into new care settings and the addition of new provider partners.
Tripling Enrollment
The growth from 1,000 to 3,000 patients reflects broad adoption across CCN Health's full program suite. Remote Patient Monitoring and Chronic Care Management account for the largest share of enrollments, while Principal Care Management, Behavioral Health Integration, and Remote Therapeutic Monitoring contribute a growing percentage as providers recognize the clinical and financial value of specialized program tracks.
The acceleration in enrollment has been supported by two parallel trends: existing partners expanding their programs to additional facilities and patient populations, and new partners launching programs through CCN Health's turnkey deployment model. Multi-site senior living operators in particular have driven significant growth by extending successful pilot programs across their facility portfolios.
Expanding Care Settings
The path to 3,000 patients has brought CCN Health into care settings beyond its initial focus on skilled nursing and assisted living. Independent living communities, continuing care retirement communities, and physician practice groups now represent meaningful segments of the patient base. Each setting presents distinct monitoring requirements, from the self-directed engagement model needed in independent living to the multi-condition complexity common in CCRC populations.
This diversification has strengthened the platform's clinical workflows, as protocols developed for high-acuity skilled nursing environments inform best practices that can be adapted for lower-acuity settings, and vice versa.
Scaling Operations
Supporting a threefold increase in patient volume required deliberate operational investment. CCN Health expanded its clinical team, refined its device logistics pipeline to handle higher provisioning volumes, and enhanced its onboarding process to reduce the time between partner agreement and first patient enrollment.
