CCN Health Announces Epic EHR Integration via FHIR-Based APIs

LOS ANGELES, CA — December 3, 2025 — CCN Health today announced a FHIR-based API integration with Epic, one of the largest electronic health record platforms in the United States. The integration enables enterprise health systems and academic medical centers to deploy Remote Patient Monitoring and Chronic Care Management programs with bi-directional clinical data exchange between the CCN Health platform and Epic's EHR environment.

FHIR-Native Data Exchange

Built on FHIR R4 standards, the integration supports bi-directional data flow for RPM device readings, care management documentation, and clinical observations. Vital sign data captured by patient monitoring devices posts directly into Epic's clinical record in real time, giving physicians and care teams immediate visibility into physiological trends without leaving their primary workflow.

Care plan synchronization ensures that monitoring protocols within the CCN Health platform reflect the treatment plans documented in Epic. When a physician updates a patient's care plan, those changes propagate to the monitoring system, adjusting alert thresholds and escalation rules accordingly.

Enterprise Billing and Documentation

The integration includes support for enterprise billing workflows. CPT code documentation, time tracking records, and clinical notes generated during RPM and CCM encounters are structured for direct import into Epic's revenue cycle management tools. This reduces the manual reconciliation that billing teams typically perform when operating third-party monitoring programs alongside an enterprise EHR.
