LOS ANGELES, CA — December 18, 2025 — CCN Health today announced a cloud-to-cloud integration with athenahealth, one of the most widely used ambulatory electronic health record platforms in the country. The integration targets independent physician practices and multi-provider groups seeking to launch or expand Remote Patient Monitoring and Chronic Care Management programs with minimal IT overhead.

Cloud-to-Cloud Architecture

The integration leverages athenahealth's open API framework to establish a direct cloud-to-cloud connection between the two platforms. Patient chart data — including demographics, problem lists, medication records, and clinical observations — syncs automatically between athenahealth and the CCN Health monitoring platform. There is no on-premise middleware or VPN configuration required.

Clinical observation posting delivers RPM device readings directly into the patient's athenahealth chart as structured data. When a patient takes a blood pressure reading or steps on a connected scale at home, that measurement appears in the clinical record alongside office-visit vitals, giving providers a continuous view of patient health between appointments.

Claims-Ready Billing Documentation

For practices managing their own billing, the integration generates claims-ready documentation for RPM and CCM encounters. Time tracking records, CPT code assignments, and clinical notes are formatted to align with athenahealth's revenue cycle workflows. Care coordination documentation captures the inter-visit activities — medication reviews, care plan updates, patient outreach — that support billing under CCM and PCM codes.
